Support Workers

gap healthcare are currently seeking motivated support workers for Young People and Adults Semi Independent Units in Medway area.

Our client aim prepare young people to lead successful independent lives, by building their self-confidence, providing them with emotional and practical support and by helping them gain essential life skills.You will be responsible for:
- Safeguarding and promoting the welfare of our young people
- Producing appropriate reports including recommendation to colleagues and Case Managers and following their guidance
- Working in line with all Policies and Procedures, correctly managing inappropriate and challenging behaviour, making appropriate judgements and decisions to minimise risks.

For supporting adults duties include:
- Supporting clients with day to day living
- Budgeting and achieving goals set out in their support plan
- Report writing
- Accompanying clients to appointments etc,
- Working in partnership with external agencies both statutory and non-statutory
- Excellent communication skills.

Successful candidates must have a minimum 6 months direct experience working with this client groups and be subject to satisfactory DBS checks.

It is essential that you have references and are IT literate.

Please be aware shifts will naturally include evenings, overnight and weekends.
